چکیده

Background Late gadolinium enhancement (LGE) imaging allows assessment of focal scar [1]. An inversion recovery based sequence is commonly used to achieve suppression of healthy myocardium signal. However, the blood pool and subendocardial scar typically have similar signal, making it difficult to distinguish subendocardial scar. Several methods have been proposed to increase the blood-scar contrast including a double inversion technique [2], and T2-prepared (T2prep) sequences [3,4]. However, all these approaches suffer from either reduced SNR or reduced scar-myocardium contrast. In this work, we propose a novel pulse sequence that uses an optimized combination of an inversion pulse and a T2prep composite pulse to simultaneously null both the healthy myocardium and blood signals, producing a black-blood LGE (BB-LGE) image without losing significant scar-myocardium contrast. We also developed a quick navigation sequence, analogous to the Look-Locker, to help determine the ideal nulling time before imaging.

Dark-blood late gadolinium enhancement without additional magnetization preparation

BACKGROUND This study evaluates a novel dark-blood late gadolinium enhancement (LGE) cardiovascular magnetic resonance imaging (CMR) method, without using additional magnetization preparation, and compares it to conventional bright-blood LGE, for the detection of ischaemic myocardial scar. LGE is able to clearly depict myocardial infarction and macroscopic scarring from viable myocardium. Howev...

Comparison of T2-preparation and magnetization-transfer preparation for black blood delayed enhancement

Background A fundamental component of the CMR exam is contrast enhanced imaging, which is crucial for delineating diseased from normal tissue. Unfortunately, diseased tissue immediately adjacent to blood often is hidden since there is poor contrast between hyperenhanced tissue and bright blood. A new method recently described, Flow-Independent Dark-blood DeLayed Enhancement technique (FIDDLE), ...

Combining extracellular volume fraction imaging and T2 quantification by cardiovascular magnetic resonance in patients with clinically suspected myocarditis

Methods Short-axis T1 and T2 mapping were implemented into a standard CMR protocol at 1.5 Tesla in 36 patients with clinically suspected myocarditis. T1 quantification was performed for ECV calculation using the modified Look-Locker inversion-recovery (MOLLI) sequence before and 15 minutes after administration of 0.075 mmol/kg gadolinium BOPTA. T2 quantification was performed using a free-breat...

Combined saturation/inversion recovery sequences for improved evaluation of scar and diffuse fibrosis in patients with arrhythmia or heart rate variability.

PURPOSE To develop arrhythmia-insensitive inversion recovery sequences for improved visualization of myocardial scar and quantification of diffuse fibrosis. METHODS A novel preparation pre-pulse, called saturation pulse prepared heart-rate-independent inversion recovery, is introduced, which consists of a combination of saturation and inversion pulses to remove the magnetization history in ea...

Impact of Primary Coronary Angioplasty Delay on Myocardial Salvage, Infarct Size, and Microvascular Damage in Patients With ST-Segment Elevation Myocardial Infarction

Methods Seventy patients with STEMI successfully treated with primary percutaneous coronary intervention within 12 h from symptom onset underwent CMR 3 2 days after hospital admission. Patients were subcategorized into 4 time-to-reperfusion (symptom onset to balloon) quartiles: 90 min (group I, n 19), 90 to 150 min (group II, n 17), 150 to 360 min (group III, n 17), and 360 min (group IV, n 17)...
